Outpatient services can be booked at the Reception Office desk on the first floor at the times indicated below, or by contacting the following addresses: GAIA, our virtual assistant, will answer you and will collect your request and you will be contacted by an operator on the same day to complete the booking

Information

During the booking process

The following information will be provided at the time of booking:

  • Date and time of the appointment;
  • Name of the doctor, if his/her identification is allowed;
  • Any medical preparation for the performance of the service;
  • Ticket or cost of the service, for self-payers;
  • Obligation on the part of the citizen who does not intend to use the booked service, to communicate the renunciation immediately and in any case no later than three days before the appointment.

The patient will receive a summary email with all the useful information relating to the appointment. A reminder text message is also sent two days before the appointment and a notification arrives for those who have the APP.

To book an outpatient visit or service in agreement with the national health system, it is necessary to have:

  • Prescription on a regional prescription book by the General Practitioner or Paediatrician of Free Choice, by a Specialist of an accredited Public or Private Facility;
  • Health Card;
  • Tax code and valid identification document, as an alternative to the Health Card.

At the outpatient clinics of the Villa Anna Nursing Home, “private” outpatient visits and services are also provided and can be used for a full fee, according to pre-established rates. The outpatient service ticket must be paid before the service is provided, at the reception desk.

Cancellation of an appointment must be made by telephone or at the counter at least 2 days before the date of the appointment.

Waiting lists must be documentable.

In addition, the “Villa Anna” Nursing Home undertakes to respect the waiting times according to the criteria dictated by the Marche Region implemented by the Nursing Home.

MAXIMUM WAITING TIMES:

  • Time to issue reports: 2 days;
  • Delivery of medical records: within the terms of the law;
  • Scheduled hospitalization: within the terms of the law;
  • Specialist and diagnostic services: on average 20 days.

At the Reception Office it is possible to pay tickets or amounts relating to services, request copies of medical records and obtain all general information relating to the activities of the Clinic.
The payment of services can be made by any means, with the limits imposed by Legislative Decree 21 Nov. 2007 n. 231 and subsequent amendments.

Please also note that the results of the tests must be collected within 90 days from the date of the service.
After this deadline, citizens will be required to pay the service in full, regardless of any exemption possessed.

Medical records
After discharge, the User can request a photocopy of the medical record from the Records Desk of the Health Directorate, at the Admission Office, on the following days:

  • Monday-Friday : 07:30-19:00
  • Saturdays and holidays: 07:30-13:30

For ordinary requests (processing within the terms of the law) of each medical record, the reimbursement of expenses set by the Administration is equal to € 20.00 + shipping costs.

It is suggested, if you are interested in having a copy of the Notice, to request it at the time of discharge at the counter, without prejudice to what is specified below.

The request for the Medical Record of hospitalization must be presented personally by the patient, showing an identity document; If the patient cannot appear in person, he or she can submit a request for a copy on the appropriate form or on plain paper, by written proxy to a trusted person.
The delegate will submit the request of the holder of the medical record, showing both his identity document and that of the person in charge of the record.

The holders of the right are parents who can request the medical record by showing an identity document and issuing a special self-certification relating to the family relationship and the right to exercise parental authority;

On request, it can be made by one of the heirs, upon presentation of an affidavit showing the names of the heirs or a substitute deed of notoriety certifying their status as heir, and an identity document of the applicant.

It is possible to request it on the 1st floor, at the Reception Office.
